By filling out this application, I understand if authorized, D’Addario prohibits the representation of D’Addario products not otherwise set forth herein. This authorization shall remain in effect, renewing annually on its anniversary date, unless terminated by either party providing the other party with written notice of its desire to terminate this authorization. D’Addario reserves the right to terminate this authorization for any reason. Upon termination, a dealer’s remaining inventory shall only be sold within its particular store and not upon any other platform. Please notify us to update your application should any of this information change.
